The certificate you obtained for the level you are claiming, and a short signed application letter to NIMC, along with your NIN and old NIN slip.
My certificate name differs from my NIN name. What then?
Fix the name on your NIN first, or the office is likely to turn the request back. A NIN name change is its own service on Docufy.
